Forms & Report Requests

     The following forms are available for download. They are PDF (portable document format) files. Please click on the desired link and print out the form, complete and return to the Sandown Police Department, or State of New Hampshire (Non-Resident Pistol Permit Application). We will not accept these forms e-mailed to us. If we have any questions for you, we need to address them when the paperwork is received.

For NH Pistol Permit Applications:

Vacation / House Checks:

     As a service to the community, the Sandown Police Department provides vacation and vacant house checks. When this form is completed and turned in at the Station, Officers will check your residence to make sure that all the doors are locked and have not been tampered with. Also, we will contact you if there is anything out of place at your residence. Please note that the frequency of the checks may vary due to call volume.

Alarm Information:

     If you have an alarm at your residence, we request that you complete our Alarm Information Sheet. This will enable us to make notifications if an alarm should be activated at your residence. Please print and complete this form so we can get a hold of you should the need arise.

Copy of an Investigative, Incident, Accident or Arrest Report or any Audio / Video Media:

     We have a form that must be completed for our record keeping. Also, please note that there is a $15.00 fee for each item requested. This fee is due with the completed Report Request Form and it may take up to a week to collect and copy the requested report(s) and or media. Please be advised, Body Camera video is not considered available for release under RSA 91:A 5 X unless it meets certain exceptions, such as:

  1. Any restraint or use of force by a law enforcement officer; provided, however, that this exemption shall not include those portions of recordings which constitute an invasion of privacy of any person or which are otherwise exempt from disclosure.
  2. The discharge of a firearm, provided that this exemption shall not include those portions of recordings which constitute an invasion of privacy of any person or which are otherwise exempt from disclosure.
  3. An encounter that results in an arrest for a felony-level offense, provided, however, that this exemption shall not apply to recordings or portions thereof that constitute an invasion of privacy or which are otherwise exempt from disclosure.

Motor Vehicle Accident Reports:

     If you have been involved in an accident and would like a copy of your report, please note that your report will not be available until at least a week after the accident occurred.  Aside from completing the report, they must be checked over and approved for completeness and accuracy before they are released.
