The following forms are available for download. They are PDF (portable document format) files. Please click on the desired link and print out the form, complete and return to the Sandown Police Department, or State of New Hampshire (Non-Resident Pistol Permit Application).  We will not accept these forms e-mailed to us. If we have any questions for you, we need to address them when the paperwork is received.

Vacation / House Check Form
As a service to the community, the Sandown Police Department provides vacation and vacant house checks. When this form is completed and turned in at the Station, Officers will check your residence to make sure that all the doors are locked and have not been tampered with. Also, we will contact you if there is anything out of place at your residence. Please note that the frequency of the checks may vary due to call volume. Click here for the form.

Alarm Information Sheet:
If you have an alarm at your residence, we request that you complete our Alarm Information Sheet. This will enable us to make notifications if an alarm should be activated at your residence. Please print and complete this form so we can get a hold of you should the need arise. Please click here for this form.

Copy of an Investigative, Incident, Accident or Arrest Report or any Audio or Video Media:
We have a form that must be completed for our record keeping. Also, please note that there is a $15.00 fee for each report requested. This fee is due with the completed Report Request Form and it may take up to a week to collect and copy the requested report(s) and or media.  Click here for the form.

Motor Vehicle Accident Reports:

New Hampshire Governor Sununu signed House Bill 437, which includes the modification to RSA 260:14 by adding the following paragraphs:

VII-a.  Nothing in this section shall prohibit a law enforcement agency of a political subdivision or its employees from releasing the following:
(a)  Copies of reports of motor vehicle accidents prepared by the agency, and filed with the division pursuant to RSA 264:25 and RSA 264:26, to an owner, operator, or passenger of a vehicle involved in said accident, pedestrian hit by a vehicle in said accident, owner of property damaged in said accident, or the insurance company of any of the foregoing parties.  Such agency may charge a reasonable fee therefor, to be deposited into the general fund of said political subdivision.
(b)  Copies of reports of motor vehicle accidents prepared by the agency that are not required to be reported pursuant to RSA 264:25 and RSA 264:26, to an owner, operator, or passenger of a vehicle involved in said accident, pedestrian hit by a vehicle in said accident, owner of property damaged in said accident, or the insurance company of any of the foregoing parties.  Such agency may charge a reasonable fee therefor, to be deposited into the general fund of said political subdivision.
(c)  Information obtained by the law enforcement agency that accident participants are required to exchange pursuant to RSA 264:25, to an owner, operator, or passenger of a vehicle involved in said accident, pedestrian hit by a vehicle in said accident, or owner of property damaged in said accident.

In short, we are now allowed to release accident reports to the involved parties or their insurance companies. If you have been involved in an accident and would like a copy of your report, please note that your report will not be available until a week after the accident occurred. Aside from completing the report, they must be checked over and approved for completeness and accuracy before they are released.
